2. The specific right(s) you wish to exercise
3. Any details needed to verify your identity

You may designate an authorized agent to submit requests on your behalf. We will require written permission from you authorizing the agent to act on your behalf, and we may verify your identity directly with you to confirm the request is legitimate.

We will disclose, upon request, the categories of personal information collected in the past 12 months and our data practices. This disclosure is available at no charge and will be delivered in a portable and readily usable format where technically feasible.
